Reverse Shoulder Replacement

When the rotator cuff can no longer do its job.

A reverse replacement swaps the geometry of the joint so the deltoid lifts the arm instead of the rotator cuff — restoring function where a cuff tear cannot be repaired.

Surgical Steps (Overview)

1
Planning

CT planning accounts for bone loss and sets implant position.

2
Approach

A deltopectoral or superolateral approach exposes the joint.

3
Glenoid Baseplate

A baseplate and glenosphere are fixed to the socket.

4
Humeral Component

The humerus is prepared and the tray implanted at planned height.

5
Tension Check

Deltoid tension and stability are verified through the arc.

Recovery Timeline (Step by Step)

Week 1–2Sling · Gentle assisted motion · Pain control
Week 3–6Progressive assisted and active motion · Sling weaned
Week 6–12Active motion against gravity · Deltoid strengthening
Month 3–6Functional strengthening · Daily tasks and overhead reach
Month 6–12Settled function · Light sport and activity*
Overview

When the rotator cuff is irreparably torn, an anatomic replacement fails because nothing holds the ball in the socket. Reversing the geometry lets the deltoid become the lifting muscle.

It is the standard solution for cuff-tear arthropathy, massive irreparable tears, failed previous replacements and certain complex fractures — and it is highly reliable at restoring overhead function.

Is a reverse shoulder replacement right for you?
  • Massive, irreparable rotator cuff tear with weakness
  • Cuff-tear arthropathy on imaging
  • Inability to lift the arm overhead (pseudoparalysis)
  • A failed previous shoulder replacement or complex fracture

Preparing for surgery
  • CT planningBone loss and implant position are planned in three dimensions
  • Medical work-upHealth, dental and medication review before admission
  • Set expectationsExcellent for lifting and comfort; rotation gains are more modest
  • Home supportArrange help for two to three weeks of one-handed living

The reverse replacement changed what we can offer patients who had simply been told to live with a useless arm. It is not a cuff repair substitute — it is a different mechanical solution, and a dependable one.

Questions & Answers

Will I be able to lift my arm again?
In most patients yes — this is the operation’s greatest strength. Forward elevation typically improves substantially.
What about rotation behind my back?
Internal and external rotation improve less predictably than elevation, and this is discussed before surgery.
How long does it last?
Ten-year survival is good and continues to improve with modern designs and planning.
What are the limits afterwards?
Heavy lifting, impact and contact sport are discouraged to protect the implant.
Can it dislocate?
Instability is uncommon and depends on correct tensioning and implant position — a key part of the operation.
Is it suitable after a failed replacement?
Yes, it is one of the most common revision solutions when the cuff is deficient.
